The employees used a forklift and lifting straps to move the machine. The machine tipped, pinning one employee against the wall of the facility. The employee suffered a broken ankle and cracked rib, as well as a possible concussion.

The employee was hospitalized with fractured ribs, a potential hole in the lung, and a facial laceration.

The employee attempted to adjust one of four legs that support the motor pack for the skip hoist, which is a separate piece from the skip hoist. The motor pack weighed 1,850 pounds. The employee was loosening a clamp on one of the motor pack legs when the leg collapsed, causing the motor pack to fall on the employee, who suffered broken vertebrae.
